The following forms are to be submitted to the Foundation Office for approval and processing:
The Fund Raising Request Form is to be completed and submitted to the Foundation for approval before planning a fund raising activity or event. An approved electronic copy will be returned to the department, the Business Office staff, and event coordinators. All information must be completed in order to expedite the approval process. The original forms are kept on file in the Foundation.
The In-Kind Gift Report Form is to be completed and submitted to the Foundation with documentation to support the value before gifts are accepted. After approval, the gifts will be entered and an acknowledgement letter will be sent to the donor by the Foundation. An approved electronic copy will be sent to the appropriate administrative office so that the gift may then be accepted. The original forms are kept on file in the Foundation.
