Documentation may be submitted at any RSCC location.
Examples include (but not limited to):
- Physician’s statement
- Hospital invoice (billing)
- Police Report
- Statement from employer
- Obituary/death certificate
- Birth certificate
- Statement from childcare provider
- Auto Repair invoice
- Legal documents from attorney (divorce papers, etc)
- Military documents (DD214, deployment papers)
Documentation must include the following:
- Provided on company letterhead
- Dates that correspond with semester you lost eligibility
- Signed by company official
- Includes phone number of company contact for verification purposes
