Under “Teaching” rank all nominees from #1 as the most outstanding to #14 as the least outstanding. Circle the appropriate rank number on the evaluation form.

More than one person may receive the same rank, based on but not limited to documented evidence from nominees. The evidence compared with but not limited to the items outlined on this instrument should support your ranking.

Follow the same procedure for ranking any two of the following three categories: Institutional Service, Community Service, and Professional Activities.

Each committee member is encouraged to complete the ranking forms by WORKING ALONE AND IN PRIVATE.

For each nominee, the weighted ranks from each committee member will be added.

The nominee with the SMALLEST sum of weighted ranks will be the recipient.

Nominees were asked to submit and label accordingly the following items which you are to evaluate:

1. Faculty Evaluation Agreement (MBOs or IEPDs) for the previous spring and fall terms.
2. Faculty Evaluation Summary Sheet including student evaluation scores for the previous spring and fall terms.
3. A narrative (up to two double-spaced pages with one inch margins on all sides using 10 point or larger font size) describing their non-teaching activities (institutional service, community service, and/or professional development for the previous spring and fall terms).
4. A two-page narrative (double-spaced with one inch margins on all sides using 10 point or larger font size) describing their teaching activities for the previous spring and fall terms. 

Each nominee was asked to place materials in a portfolio and label the content as listed above in A through D.

After the last meeting, please return all portfolios to the Benroth Committee Chairperson.
